ABSTRACT

Abstract Brazil has a huge number of cases and deaths due to coronavirus disease 2019 (COVID-19); however, few studies have dealt with severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection among familial contacts in Brazil. Here, we report our findings on transmission in a family-based study in Bauru, São Paulo, Brazil. The study, conducted from July to November 2020, comprised 974 individuals with 233 index patients and 741 familial contacts. Familial contacts were evaluated using the rapid COVID-19 Ag ECO and reverse transcription-polymerase chain reaction (RT-PCR) tests immediately after the index patient diagnosis. The antigen-based rapid test was validated in 121 individuals using RT-PCR as the gold standard. Additionally, 30 days later, familial contacts were evaluated for IgM and IgG antibodies against SARS-CoV-2. We found 333 cases of COVID-19 among familial contacts (44.9%). A positive correlation was observed between the time elapsed from the onset of symptoms until the index patient's COVID-19 testing and the number of family contacts infected by SARS-CoV-2. Early SARS-CoV-2 testing and familial contact evaluation are relevant strategies to contain transmission.


Resumo O Brasil apresenta um alto número de casos e óbitos por coronavírus (COVID-19), apesar disso, poucos estudos tratavam da infecção pelo coronavirus-2 causador de síndrome respiratória aguda grave (SARS-CoV-2) entre contatos familiares no Brasil. Relatamos aqui nossos achados sobre a transmissão de SARS-CoV-2 em um estudo de base familiar de Bauru, no estado de São Paulo, Brasil. O estudo foi realizado de julho a novembro de 2020 e compreendeu 974 indivíduos, sendo 233 pacientes índice e 741 contatos familiares. Os contatos familiares foram avaliados por meio do teste rápido COVID-19 Ag ECO Test e RT-PCR imediatamente após o diagnóstico do paciente índice. O uso do teste rápido baseado em antígeno foi validado em 121 indivíduos utilizando RT-PCR como padrão ouro. Adicionalmente, 30 dias após a avaliação inicial, os contatos familiares foram avaliados quanto à presença de anticorpos IgM e IgG contra SARS-CoV-2. Encontramos 333 casos de COVID-19 entre contatos familiares (44,9%). Observamos uma correlação positiva entre o tempo decorrido entre o início dos sintomas e o teste para COVID-19 do paciente índice e o número de contatos familiares infectados por SARS-CoV-2. A testagem precoce da infecção por SARS-CoV-2 e a avaliação de contatos familiares são estratégias relevantes para conter a transmissão.

BACKGROUND Leprosy, caused by Mycobacterium leprae, is a public health problem in Brazil that affects peripheral nerves, resulting in physical disabilities. During host-pathogen interactions, the immune response determines leprosy outcomes from a localised (paucibacillary) form to a disseminated (multibacillary) form. The recognition of M. leprae involves the DC-SIGN receptor, which is present on the dendritic cells (DCs) and participates in immune activation. OBJECTIVES To evaluate the association of polymorphisms in the promoter region of the gene encoding DC-SIGN (CD209) and the clinical form of leprosy, and to investigate its functional effects. METHODS The study population included 406 leprosy patients from an endemic area in Brazil [310 multibacillary (MB); 96 paucibacillary (PB)]. A functional evaluation based on the effects of the single nucleotide variant (SNV) associated with PB leprosy on the specific immune response was also performed. RESULTS The GA genotype and the presence of the A allele of rs735240 (-939G>A) were associated with PB leprosy [OR: 2.09 (1.18-3.69) and 1.84 (1.07-3.14), respectively]. Carriers of the A allele showed reduced expression of CD209 and TGF-β1 in leprosy lesions in comparison with individuals with GG genotype, in addition to a higher response to the Mitsuda test. CONCLUSION These data suggest that rs735240 influences the immune response against M. leprae and clinical presentation of leprosy.

BACKGROUND Leprosy is a chronic infectious disease caused by Mycobacterium leprae, and compromises the skin and peripheral nerves. This disease has been classified as multibacillary (MB) or paucibacillary (PB) depending on the host immune response. Genetic epidemiology studies in leprosy have shown the influence of human genetic components on the disease outcomes. OBJECTIVES We conducted an association study for IL2RA and TGFB1 genes with clinical forms of leprosy based on two case-control samples. These genes encode important molecules for the immunosuppressive activity of Treg cells and present differential expressions according to the clinical forms of leprosy. Furthermore, IL2RA is a positional candidate gene because it is located near the 10p13 chromosome region, presenting a linkage peak for PB leprosy. METHODS A total of 885 leprosy cases were included in the study; 406 cases from Rondonópolis County (start population), a hyperendemic region for leprosy in Brazil, and 479 cases from São Paulo state (replication population), which has lower epidemiological indexes for the disease. We tested 11 polymorphisms in the IL2RA gene and the missense variant rs1800470 in the TGFB1 gene. FINDINGS The AA genotype of rs2386841 in IL2RA was associated with the PB form in the start population. The AA genotype of rs1800470 in TGFB1 was associated with the MB form in the start population, and this association was confirmed for the replication population. MAIN CONCLUSIONS We demonstrated, for the first time, an association data with the PB form for a gene located on chromosome 10. In addition, we reported the association of TGFB1 gene with the MB form. Our results place these genes as candidates for validation and replication studies in leprosy polarisation.

Abstract: This study aimed to identify the distribution pattern of leprosy in a hyperendemic municipality in Brazil and determine its relationship with the clinico-epidemiological situation over 11 years. The geographic information system, MapInfo, spatial scan statistics and the Moran I index were used to analyze new cases. The digital cartographic base was used to map clusters of new paucibacillary and multibacillary cases and cases in minors under 15 years old. Socioeconomic indicators are shown using the choropleth mapping technique. A reduction in the detection coefficient, increases in high-risk spatial clusters, marked changes in the distribution of high-risk and low-risk clusters, and high-risk clusters of minors under 15 years old were observed from 2006 to 2010, showing recent illness, the presence of active foci, and overlapping of high-risk clusters of multibacillary infection in minors under 15 years old. Leprosy remains a public health problem in Rondonópolis, Mato Grosso State; the high-risk areas require an intensification of control measures and active search strategies to detect new cases.


Resumo: O estudo teve como objetivos identificar o padrão de distribuição da hanseníase em um município brasileiro hiperendêmico e determinar a relação com o quadro clínico-epidemiológico ao longo de 11 anos. Os casos novos foram analisados com o sistema de informação geográfica, MapInfo, estatística scan espacial e índice Moran I. A base cartográfica digital foi usada para mapear os clusters de casos paucibacilares e multibacilares novos e casos em menores de 15 anos. Os indicadores socioeconômicos são mostrados através da técnica de mapeamento coroplético. Entre 2006 e 2010, foram observados uma redução no coeficiente de detecção, aumento no clusters espaciais de alto risco, mudanças marcantes na distribuição de clusters de alto e baixo risco e clusters de alto risco em menores de 15 anos, sugerindo doença recente, a presença de focos ativos e a sobreposição de clusters de alto risco para infecção multibacilar em menores de 15 anos. A hanseníase persiste enquanto problema de saúde pública em Rondonópolis, Mato Grosso; as áreas de alto risco exigem a intensificação de medidas de controle, além de estratégias de busca ativa para detectar casos novos.


Resumen: Este estudio tuvo como objetivo identificar la distribución de los patrones de lepra en una municipalidad hiperendémica en Brasil y determinar su relación con la situación clínico-epidemiológica durante 11 años. El sistema de información geográfica, MapInfo, estadísticas de escaneo espacial y el índice de Moran se usaron para analizar nuevos casos. La base cartográfica digital se usó para mapear clústeres de nuevos casos multibacilares y paucibacilares, así como casos en menores por debajo de 15 años de edad. Los indicadores socioeconómicos se presentan usando la técnica de mapeo de coropletas. La reducción en la detección del coeficiente, se incrementa en los clústeres de alto riesgo espaciales, asimismo, se observaron de 2006 a 2010 cambios considerables en la distribución de los clústeres de alto riesgo y bajo riesgo, así como en clústeres de alto riesgo con menores con menos de 15 años de edad, mostrando los casos de enfermedad reciente la presencia de focos activos, así como solapando clústeres de alto riesgo de infección multibacilar en menores por debajo de los 15 años de edad. La lepra continúa siendo un problema de salud pública en Rondonópolis, Mato Grosso; las áreas de alto riesgo necesitan una intensificación de las medidas de control y una búsqueda activa de estrategias, con el fin de detectar nuevos casos.

ABSTRACT

Abstract Leprosy, whose etiological agent is Mycobacterium leprae, is a chronic infectious disease that mainly affects the skin and peripheral nervous system. The diagnosis of leprosy is based on clinical evaluation, whereas histopathological analysis and bacilloscopy are complementary diagnostic tools. Quantitative PCR (qPCR), a current useful tool for diagnosis of infectious diseases, has been used to detect several pathogens including Mycobacterium leprae. The validation of this technique in a robust set of samples comprising the different clinical forms of leprosy is still necessary. Thus, in this study samples from 126 skin biopsies (collected from patients on all clinical forms and reactional states of leprosy) and 25 slit skin smear of leprosy patients were comparatively analyzed by qPCR (performed with primers for the RLEP region of M. leprae DNA) and routine bacilloscopy performed in histological sections or in slit skin smear. Considering clinical diagnostic as the gold standard, 84.9% of the leprosy patients were qPCR positive in skin biopsies, resulting in 84.92% sensitivity, with 84.92 and 61.22% positive (PPV) and negative (NPV) predictive values, respectively. Concerning bacilloscopy of histological sections (BI/H), the sensitivity was 80.15% and the PPV and NPV were 80.15 and 44.44%, respectively. The concordance between qPCR and BI/H was 87.30%. Regarding the slit skin smear, 84% of the samples tested positive in the qPCR. Additionally, qPCR showed 100% specificity, since all samples from different mycobacteria, from healthy individuals, and from other granulomatous diseases presented negative results. In conclusion, the qPCR technique for detection of M. leprae using RLEP primers proved to be specific and sensitive, and qPCR can be used as a complementary test to diagnose leprosy irrespective of the clinical form of disease.

SUMMARY The aim of this study was to evaluate the effects of the protein-calorie malnutrition in BALB/c isogenic mice infected with Lacazia loboi, employing nutritional and histopathological parameters. Four groups were composed: G1: inoculated with restricted diet, G2: not inoculated with restricted diet, G3: inoculated with regular diet, G4: not inoculated with regular diet. Once malnutrition had been imposed, the animals were inoculated intradermally in the footpad and after four months, were sacrificed for the excision of the footpad, liver and spleen. The infection did not exert great influence on the body weight of the mice. The weight of the liver and spleen showed reduction in the undernourished groups when compared to the nourished groups. The macroscopic lesions, viability index and total number of fungi found in the footpads of the infected mice were increased in G3 when compared to G1. Regarding the histopathological analysis of the footpad, a global cellularity increase in the composition of the granuloma was observed in G3 when compared to G1, with large numbers of macrophages and multinucleated giant cells, discrete numbers of lymphocytes were present in G3 and an increase was observed in G1. The results suggest that there is considerable interaction between Jorge Lobo's disease and nutrition.


RESUMO O objetivo do estudo foi avaliar os efeitos da desnutrição protéico-calórica em camundongos isogênicos da linhagem BALB/c inoculados com Lacazia loboi, empregando parâmetros nutricionais e histopatológicos. Foram constituídos quatro grupos: G1- inoculados com restrição dietética; G2- não inoculados com restrição dietética; G3- inoculados sem restrição dietética; G4- não inoculados sem restrição dietética. Após instalada a desnutrição, os animais foram inoculados via intradérmica no coxim plantar e após quatro meses foram sacrificados para remoção do coxim plantar, fígado e baço. A infecção não exerceu grande influência no peso corporal dos camundongos. O peso do fígado e baço apresentou redução nos grupos desnutridos em comparação aos grupos nutridos. A lesão macroscópica, a viabilidade e o número total de fungos dos coxins plantares dos camundongos inoculados revelaram aumento no G3 quando comparado com o G1. Em relação à análise histopatológica dos coxins plantares observou-se aumento da celularidade global na composição do granuloma no G3 em relação ao G1, com grande número de macrófagos e células gigantes multinucleadas, discretos números de linfócitos estavam presentes em G3 e aumentados no G1. Os resultados sugerem que existe grande interação entre nutrição e doença de Jorge Lobo.

Jorge Lobo's disease is a cutaneous and subcutaneous mycosis that affects patients in the Amazon region. The number of patients is relatively small, but the real situation of the disease as public health problem is not known, because Jorge Lobo's disease is not a notifiable disease. This study aims to report the clinical evolution in patients affected and to determine the prevalence and areas of occurrence of the disease. A retrospective study was carried out based on the analysis of the clinical records, which included a collection of photographs of patients in the Department of Sanitary Dermatology, in Rio Branco, and patients seen in the interior of the state. In a decade, in Rio Branco, 249 cases of the disease were reported, 30 were females and 219 males. Of these patients, 153 had localized lesions, 94 of them were on one ear, 55 had multifocal lesions and 41 had disseminated lesions. The average time between the onset of symptoms and diagnosis was 19 years. The average age at the time of diagnosis was 53 years, and ages ranged from 14 to 96 years.


A doença de Jorge Lobo é micose cutânea e subcutânea que afeta pessoas na região Amazônica. O número de pacientes é relativamente pequeno, no entanto, a real prevalência da doença como problema de saúde pública é pouco conhecida. A doença de Jorge Lobo não é de notificação compulsória. Este estudo teve como objetivo determinar a prevalência, as áreas de ocorrência da doença de Jorge Lobo, além de sua evolução clínica. Um estudo retrospectivo foi desenvolvido com base na análise de prontuários de pacientes, incluindo documentação fotográfica dos mesmos, que foram atendidos no Departamento de Dermatologia Sanitária em Rio Branco e no interior do Estado. Foram registrados 249 casos em uma década em Rio Branco, 30 mulheres e 219 homens. Do total 153 apresentavam lesões localizadas, 94 lesões em apenas uma orelha, 55 lesões multifocais e 41 lesões disseminadas. A média de tempo entre o início dos sintomas e o diagnóstico foi de 19 anos. A média de idade no momento do diagnóstico foi de 53 anos, e as idades variaram de 14 a 96 anos.

Since there are no studies evaluating the participation of the complement system (CS) in Jorge Lobo's disease and its activity on the fungus Lacazia loboi, we carried out the present investigation. Fungal cells with a viability index of 48 percent were obtained from the footpads of BALB/c mice and incubated with a pool of inactivated serum from patients with the mycosis or with sterile saline for 30 min at 37 °C. Next, the tubes were incubated for 2 h with a pool of noninactivated AB+ serum, inactivated serum, serum diluted in EGTA-MgCl2, and serum diluted in EDTA. The viability of L. loboi was evaluated and the fungal suspension was cytocentrifuged. The slides were submitted to immunofluorescence staining using human anti-C3 antibody. The results revealed that 98 percent of the fungi activated the CS by the alternative pathway and no significant difference in L. loboi viability was observed after CS activation. In parallel, frozen histological sections from 11 patients were analyzed regarding the presence of C3 and IgG by immunofluorescence staining. C3 and IgG deposits were observed in the fungal wall of 100 percent and 91 percent of the lesions evaluated, respectively. The results suggest that the CS and immunoglobulins may contribute to the defense mechanisms of the host against L. loboi.


Considerando que não existe nenhum estudo avaliando a participação do sistema complemento (SC) na doença de Jorge Lobo e sua atividade sobre o fungo Lacazia loboi, realizamos o presente trabalho. Os fungos foram obtidos dos coxins plantares de camundongos BALB/c com índice de viabilidade de 48 por cento e, em seguida, foram incubados com pool de soro inativado de pacientes ou com solução salina estéril (SSE) por 30 min, a 37 °C. Os tubos foram incubados, por 2 h, com pool de soro AB+ sem inativar, inativado, diluído em EGTA-MgCl2 e EDTA. A viabilidade do L. loboi foi avaliada e a suspensão fúngica foi citocentrifugada. As lâminas foram submetidas à técnica de imunofluorescência empregando o anticorpo anti-C3 humano. Os resultados revelaram que 98 por cento dos fungos ativaram o SC pela via alternativa e que não houve diferença significante na viabilidade do L. loboi após ativação do SC. Em paralelo, cortes histológicos congelados de 11 pacientes foram avaliados quanto à presença de C3 e IgG, pela técnica de imunofluorescência. Foram encontrados depósitos de C3 e de IgG na parede dos fungos em 100 por cento e 91 por cento das lesões avaliadas, respectivamente. Os resultados sugerem que o SC e as imunoglobulinas poderiam contribuir nos mecanismos de defesa do hospedeiro contra o L. loboi.

ABSTRACT

A doença de Jorge Lobo é uma micose cutânea e subcutânea de evolução crônica, causada pelo Lacazia loboi. Os mecanismos envolvidos na suscetibilidade/resistência do hospedeiro frente ao agente não estão ainda elucidados, dentre eles encontra-se a constituição genética. Portanto, o objetivo deste trabalho é avaliar a distribuição fenotípica do grupo sangüíneo Duffy em pacientes com doença de Jorge Lobo e calcular suas freqüências gênicas. Foram analisados 21 pacientes, 13 caucasóides e oito mestiços (tipo moreno claro), provenientes do estado do Acre, e 44 indivíduos, não acometidos por esta doença, habitantes da cidade de Rio Branco (AC), como grupo controle. O fenótipo Duffy foi definido pela presença dos antígenos nas hemácias pela reação de aglutinação dos eritrócitos com anti-soros anti-Fya e anti-Fyb incubando-se a 37ºC e aplicando-se o teste da antiglobulina humana. Os resultados mostram que a distribuição fenotípica do sistema Duffy nos pacientes difere significativamente daquela observada nos controles, pela alta freqüência de indivíduos com fenótipo Fy(a-b-). Observa-se claramente que os pacientes apresentam maior freqüência do alelo FY e menor do alelo FYA do que os controles. O alelo FYB (não inclui os alelos que não são expressos) tem praticamente a mesma freqüência nos dois grupos. Com relação à comparação entre distribuição do grupo sangüíneo Duffy com as formas clínicas, sugere maior número de pacientes com forma localizada da doença de Jorge Lobo apresentando um predomínio do fenótipo Fy(a-b+) seguido do fenótipo Fy(a-b-) e inexistência do fenótipo Fy(a+b-). Por outro lado, temos a forma disseminada com maior incidência de fenótipo Fy(a+b-). A forma multifocal foi menos expressiva. Desse modo, podemos inferir uma provável prevalência de fenótipo Duffy negativo na região por nós estudada e os poucos dados disponíveis até o momento tornam clara a necessidade de elaborar tabelas de freqüências fenotípicas para a população...


Jorge Lobo's disease is a chronic cutaneous-subcutaneous mycosis caused by the fungus Lacazia loboi. The mechanisms involved in susceptibility and resistance to infection by this pathogen, especially genetic background, are yet to be understood. The objective of the present study was to evaluate the phenotypic distribution of the Duffy blood group in Jorge Lobo's disease patients and calculate its gene frequencies. Twenty one patients were analyzed, among them 13 Caucasoid and 8 from mixed ethnical backgrounds (Afro-Brazilians) originating from the State of Acre. The control group consisted of 44 healthy individuals from the city of Rio Branco (AC). The Duffy phenotype was defined by the presence of antigens in erythrocytes as visualized by the agglutination reaction of erythrocytes in the presence of anti-Fya and anti-Fyb antisera incubated at 37ºC by the human antiglobulin test. Results showed that the phenotypic distribution of the Duffy blood group system in patients was significantly different from controls, with the frequency of individuals with phenotype Fy(a-b-) being very high. Patients presented remarkably higher frequencies of the allele FY and ower frequencies of the allele FYA compared to controls. Frequencies of the allele FYB (not including non-expressed alleles) were similar in both groups. The comparison between Duffy blood group distribution with clinical forms of the disease suggests predominance of the phenotype Fy(a-b+) in patients with the localized form of Jorge Lobo's disease, followed by phenotype Fy(a-b-) and absence of phenotype (a+b-). On the other hand, we observed a higher incidence of phenotype Fy(a+b-) in patients with the disseminated form of the disease. The multifocal forms were less expressive. Therefore, we may infer that the negative Duffy phenotype is probably more prevalent in the region studied. ABSTRACT

FUNDAMENTOS: Na hanseníase dimorfa é comum a ocorrência de reações tipo 1 antes, durante ou depois da poliquimioterapia (PQT). Trabalhos recentes sugerem que a reação tipo 1 seria um desequilíbrio imunológico entre citocinas pró-inflamatórias e antiinflamatórias. OBJETIVOS: Compreender melhor a fisiopatologia das reações tipo 1. MÉTODOS: Estudaram-se biópsias cutâneas de 10 indivíduos com hanseníase dimorfa-tuberculóide reacional não tratada (DTR) e 10 dimorfos em reação reversa após o início da PQT (DRR), comparando-se os parâmetros morfológicos e imunológicos por meio de colorações HE e Faraco-Fite, e técnicas imuno-histoquímicas (CD4, CD8, CD20, CD79a, CD57, iNOS, IL-10, LAM e BCG). RESULTADOS: Houve, nos DRR, mais macrófagos multivacuolados, maior marcação nos macrófagos para a enzima óxido nítrico sintase induzível (iNOS) e menos linfócitos T CD8+ (p<0,05). Afora a presença de bacilos típicos nos DTR e sua ausência nos DRR, não houve diferenças na baciloscopia ou na marcação para antígenos micobacterianos (LAM e BCG) entre os grupos. O número de células IL-10+ foi similar nos dois grupos, porém houve correlação negativa entre essa citocina e a proporção CD4/CD8 apenas nos pacientes DRR (p<0,05). Houve tendência à redução do infiltrado específico e ao maior número de células NK nos DRR. CONCLUSÃO: Na presença de muitos bacilos viáveis em um paciente sem imunidade celular plena, haveria tendência à piora imunológica (downgrading). A PQT, ao reduzir a carga bacilar, melhoraria a imunidade celular (upgrading), com posterior desvio da imunidade adquirida para a inespecífica (resposta Th3), evoluindo para a cura.


BACKGROUND: Type 1 reactions are common in borderline leprosy, and can occur before, during or after multidrugtherapy (MDT). Recent papers suggest that these reactions could be a result of an imbalance between proinflammatory and anti-inflammatory citokines. OBJECTIVE: To understand better the physiopathology of type 1 reactions. METHODS: We studied skin biopsies from 10 non-treated patients with reactional borderline tuberculoid leprosy (BTR) and 10 from borderline leprosy with reversal reactions after the beginning of MDT (BRR), to compare morphological and immunological parameters by routine staining (H-E and Faraco-Fite) and immunohistochemical technics (CD4, CD8, CD20, CD79a, CD57, iNOS, IL-10, LAM and BCG). RESULTS: We found, in BRR group, stronger staining for iNOS into macrofages, fewer CD8+ T cells and more multivacuolated macrofages than BTR group (p<0,05). Despite the presence of viable bacilli in BTR and its absence in BRR, there weren't differences in baciloscopy and staining for mycobacterial antigens (LAM and BCG) between the groups. The number of IL-10+ cells was similar in both groups, but there was a negative correlation between this cytokine and the CD4:CD8 ratio only in BRR group (p<0,05). It was seen a tendency for a decreased specific infiltrate and increased number of NK cells in BRR group. CONCLUSIONS: The presence of many viable bacilli in a patient with partial cellular immunity could worse the immunological status (downgrading). Once started MDT, the reduction bacilli charge would improve cellular immunity (upgrading), with latter shift to innate immunity (Th3 response), evolving to cure.

ABSTRACT

O tatu foi um modelo experimental importante para o estudo da hanseniase, alem de ser ainda uma importante fonte para coleta de bacilos. Apesar dos inumeros relatos de estudos em tatus de especie Dasypus novemcinctus, pouco se sabe sobre a real susceptibilidade desta especie ao bacilo de Hansen apos inoculaçao experimental com M. leprae. Alguns autores relatam que cerca de 80 por cento dos animais desenvolveriam a doença quando infectados. No Brasil, a inoculaçao experimental desta especie resultou em inoculaçoes positiva em apenas dois momento, tendo ate sido levantada a hipotese de estes animais serem mais resistentes a infecçao experimental. No presente estudo, utilizou-se a resposta ao nantigeno de Mitsuda como um indicador de resposta imune celular de tatus ao M. leprae...

ABSTRACT

Neste trabalho foram acompanhados clinicamente 33 pacientes com a idade de nove a 87 anos, 28 masculinos e cinco femininos, suspeitos de hanseníase, com neuropatia periférica, sem lesão de pele evidente e sem baciloscopia e biópsia de pele positivas, durante o período de 1994-2004. Todos foram submetidos aos exames dermatológico, neurológico, neurofisiológico, reação de Mitsuda e biópsia de nervo. As biópsias de nervo foram fixadas em FMA e ósmio e os cortes histológicos corados pela hematoxilina-eosina, Fite-Faraco e imunoistoquímica com anticorpo anti-BCG. O quadro clínico de polineuropatia ocorreu em 17 (51,51%) pacientes, mononeuropatia múltipla em 13 (39,39%) e mononeuropatia em 3 (9,1%). As colorações pela hematoxilina-eosina e o Fite-Faraco foram conclusivas para hanseníase em 10 (30,30%) pacientes, três deles apresentaram padrão Dimorfo, dois padrão Tuberculóide e nos restantes cinco não havia padrão histológico característico. A classificação final dependeu da correlação clínico-histológica. O estudo imunoistoquímico foi positivo em todos os casos com infiltrado inflamatório específico, e em um paciente com infiltrado inflamatório inespecífico, aumentando o diagnóstico de hanseníase para 33,33% (11). Durante o acompanhamento clínico dos 22 pacientes restantes, três tiveram o diagnóstico de hanseníase confirmado e nos demais 19 (57,57%) foi afastada a possibilidade de hanseníase, passando a 42,43% (14) os pacientes com hanseníase primariamente neural. A hanseníase primariamente neural é uma manifestação incomum da hanseníase, e o seu diagnóstico é complexo. O acompanhamento clínico em conjunto com a melhora das técnicas do exame histopatológico do nervo oferece mais acuidade para a investigação dos casos suspeitos de hanseníase primariamente neural

ABSTRACT

A total of 33 patients, 28 males and five females, from nine to 87 years of age, with suspected leprosy associated peripheral neuropathy, without detectable skin lesion or positive skin bacilloscopy, were studied during the period of 1994 to 2004. Patients were submitted to dermatological and neurological examination, electrophysiologic tests, Mitsuda reaction and nerve biopsy. Samples for histopathological exams were stained with hematoxillin-eosin, Faraco-Fite and immunohistochemistry with anti- BCG antibodies. Among patients with suspected leprosy, the clinical presentation of polyneuropathy occurred in 17 (51,51%) patients while 13 (39,39%) presented mononeuropathy multiplex and 3 (9,1%) mononeuropathy. The hematoxillin-eosin and Faraco-Fite stainings confirmed the leprosy diagnosis in 10 (30,30%) patients. Three patients presented a borderline pattern, two tuberculoid and no characteristic histological pattern was observed in the remaining five patients. The final calssification depended on the clinical-histological correlation. Immunohistochemistry increased the diagnosis to 11 (33,33%) cases. Among the remaining 22 patients, three patients had leprosy confirmed, one was clinically understood as a primarily neural leprosy, probably tuberculoid form of the childhood, increasing the diagnosis to 14 (42,43%) cases. From the remaining patients, 19 (57,57%) were excluded during the follow-up. The primarily neural leprosy (PNL) is an unusual leprosy presentation and a complex form to diagnose. The clinical follow-up accompanied by the improvement of histopathological examination of the nerve may add more accuracy to the investigation of the suspected leprosy neuropathies

ABSTRACT

In a previous study, the authors inoculated Swiss mice with Lacazia loboi (L. loboi) and succeeded in maintaining a granulomatous infiltrate and viable fungal cells up to one year and six months after inoculation. Considering the experimental work on paracoccidioidomycosis, 0.03 ml of a fungal suspension obtained from a biopsy of a Jorge Lobo's Disease patient were inoculated into both hind foot pads of 32 six week-old BALB/c mice of both sexes. The animals were sacrificed 1, 4, 7 and 10 months post inoculation. The suspension contained 1.3 x 10(6) fungi/ml and presented 38 percent viability. Seven months after inoculation, most of the animals presented profuse infiltrates consisting of isolated histiocytes, foreign body and Langhans' giant cells and a large number of fungi, most of them viable. Emergence of macroscopic lesions was observed during the 8th month. Based on fungal count, viability index before and after inoculation, presence of macroscopic lesions and histopathological findings similar to the findings in humans, the authors believe that BALB/c mice may be a good experimental model to study Jorge Lobo's Disease, mainly regarding therapeutic evaluation

Sixty-four isogenic Swiss mice were intradermically inoculated in both hind foot pads. The inocula, consisting of fungal suspensions from biopsies obtained from Jorge Lobo's Disease patients, had the total number of fungi and the viability index determined using a Neubauer chamber and the fluorescein diacetate-ethidium bromide technique (FD-EB), respectively. The animals were sacrificed at times ranging from ten days to eighteen months after inoculation. The cellular infiltrate, mainly consisting of macrophages containing fungi, increased progressively up to end of the study; however, no macroscopic alterations were observed in the inoculated feet. After nine months, small numbers of Langhans' giant cells started to appear in the infiltrate. A considerable number of fungi was observed at the end of the experimental period, but only a few were viable when stained by the FD-EB technique. This fact suggests that there is a multiplication of fungal cells, which are destroyed by the macrophages but remain in the tissue for a long time due perhaps to the difficulties in their elimination. These findings led us to conclude that in spite of the maintenance of the infection in these animals, Swiss mice cannot be considered an ideal model to study Jorge Lobo's Disease. However, the authors call attention to the possibility of other mouse strains being more susceptible to Paracoccidioides loboi.

Na literatura há referências de que a proliferaçäo do M. leprae nas vísceras pode se manter mesmo na ausência de proliferaçäo, em nível cutâneo-neural, e que isto poderia permitir reativações da hanseníase. Com o fito de avaliar-se a baciloscopia nas diversas localizações viscerais comprometidas na hanseníase virchoviana e, se possível, compará-las com a baciloscopia em material colhido em pele e nervos periféricos, analisamos 60 necrópsias de indivíduos virchovianos. Estas necrópsias foram divididas em grupos de acordo com os resultados de última baciloscopia que o paciente apresentou em vida, ou seja: virchovianos ativos e em progressäo - 15 necrópsias; Virchovianos ativos e em regressäo - 17 necrópsias; virchovianos inativos - 28 necrópsias. Em algumas necrópsias o índice da baciloscopia visceral ultrapassou o mesmo índice em nível cutâneo-neural, e em 3 necrópsias bacilos típicos foram encontrados somente em vísceras (laringe e testículos). Em termos comparativos com a baciloscopia cutâneo-neural estes resultados näo puderam ser valorizados porque limitaçöes de natureza técnica e legal nos impediram de colher fragmentos de pele e nervos periféricos em várias localizaçöes. No entanto, há trabalhos sugerindo que a baciloscopia em linfonodos de drenagem da pele pode dar uma idéia bastante apoximada da baciloscopia cutânea. Em algumas das necrópsias examinadas, em localizaçöes viscerais, o índice baciloscópico ultrapassou o mesmo índice no linfonodo axilar. Excluindo-se linfonodos axiliares, os valores mais elevados do índice baciloscópico para as vísceras foram encontrados na laringe, testículos e faringe, com predominio da primeira localizaçäo. Isto pode confirmar a relaçäo entre temperatura tecidual e capacidade de proliferaçäo bacilar, sugerindo que a laringe apresenta condiçöes de adaptaçäo e proliferaçäo do M. leprae semelhantes à mucosa nasal, que é a via principal de eliminaçäo bacilar
